How to Register

- To register for the 5th Mediterranean Congress of PRM, please complete the registration form.

- Each participant, including invited speakers, must complete a separate registration form.

- Please write/fill clearly and make sure the entire form is properly completed.

- You can register on-line or send or fax the completed form to the Congress Secretariat :

Registration For Workshop

There are 3 workshops between 08:30- 12:30 on the 30 th of September. The registration fee is 30 Euro.

Cheques:
We regret that cheques will not be accepted.

Credit Cards:
The credit card number and expiration date should be filled out in the space indicated on the registration form.
Please do not forget to sign your registration form.
On site payment:
After September 20, 2004 only Euro cash or credit card payments can be accepted.

Confirmation of Registration

After the Congress Secretariat has received your completed registration form and full registration payment, you are registered and you will receive written confirmation by e-mail. We advise you to bring a copy of the confirmation letter with you to the Congress.

Registrations made after September 20, 2004 will not be confirmed. This does not mean that you are not registered. In that case, we advise you to bring a copy of the prof of payment to the Congress.

Cancellation Policy

Requests for cancellation must be made in writing to the congress secretariat.
Refunds will be made according to the following schedule:
-Full refund for written cancellations received before june 30, 2004.(*).
-50% refund for written cancellations received before August 01, 2004.(*)
-No refund for cancellations received on or after September 01, 2004.

(*)Cancellations will be charged with administration cost Euro 30.
All refunds due to cancellation will be processed after the Congress.
